Training Documentation Requirements

Prior to the in-water training, students are required to complete a Liability Release and Assumption of Risk Acknowledgement Form stating that the understand the risks of diving and agree to assume those risks.

In addition, students also need to complete a Standard Safe Diving Practices Statement of Understanding.  This document outlines safe dive practices and reminds you of your responsibility to follow them while diving.

Both of these forms are a requirement of the Dive training certification agency.

If the student is under 18 years of age they also require the forms to be signed by their parent or guardian.
