Medical Dive Requirements
Anybody participating in an in-water training course are required to complete a Medical Questionnaire.
The purpose of this Medical Questionnaire is to find out if the student should be examined by their doctor before participating in recreational diver training. A positive response to a question does not necessarily disqualify the student from diving. A positive response means that there is a pre-existing condition that may affect their safety while diving and that they must seek the advice of their physician prior to engaging in dive activities.
Prior to commencing the course, students are required to download, complete and sign Page 1 of the Medical Questionnaire. If the student is under 18 years of age they also require the form to be signed by their parent or guardian.
If you have any additional questions regarding the Medical Statement or the Medical Questionnaire, do not hesitate to review them with your instructor before signing.
If the student answers YES to any question on Page 1, they need to complete Page 2 of the form and consult with their Doctor to obtain a Dive Clearance. Pages 3-6 of the form provide guidance for your Doctor.
Costs associated with obtaining a medical clearance from your Doctor are the responsibility of the student.
